  3. Optionally mention that you would like to be named on the Scoreboard. If you do not do this, you will be listed as Anonymous.

Upon receipt of the card with a valid return mailing address, you will be sent a check for $5.12. If you prefer we can also pay your reward via online payment services such as PayPal.
